Supported Living

DISABILITY

We have a versatile and diverse variety of disability support. We offer expert-tailored support and care to people over the age of 18 who have mild, moderate, or severe disabilities, including: Autism, sensory disabilities, physical disabilities, acquired brain injury, mental health needs and behavioral challenges

Individual needs are met through a planned care and support plan based on the Life Star model. It provides assistance in eleven areas:

  • Your well-being
  • What do you do with your free time?
  • Being responsible
  • Being safe
  • Money and letters
  • Competence in daily living
  • Communicating
  • Feeling good
  • People you know.
  • Mental health and other issues

We encourage people to develop their skills and strengths in order to live as independently as possible.

HOMELESSNESS

We assist adults who have experienced or are at risk of experiencing homelessness, as well as those who may need to overcome an addiction to addictive substances. We accomplish this in a variety of ways, including supported housing, work on preventative outreach, assistance with resettlement, and floating support services. We accomplish this through collaborative support based on the Homelessness Star model, which teaches self-confidence as well as the skills needed to become self-sufficient. Support is focused on ten areas:

  • Motivation and accountability
  • Self-care and daily living abilities
  • Money management and personal administration
  • Interpersonal relationships and social networks
  • Drug and alcohol misuse
  • Physical fitness
  • Emotional and mental well-being
  • Making good use of your time
  • Managing tenancies and accommodation
  • Offending

YOUNG PEOPLE

We assist young people aged 16 to 25 by encouraging them to focus on their talents and create positive support networks in order to achieve their goals, whether that is returning to school, finding meaningful work, or meeting new friends. Our services focus on assisting young people who are homeless, especially those who are overcoming addiction, young parents or are facing a family or relationship breakdown

Supported housing services which provide a safe place for young people to live, learn, and make the transition to independent adulthood, are among our services for young people. We also provide move-on services to assist residents in finding and maintaining their own independent housing, as well as floating support or community outreach services in which we provide one-on-one or group assistance to young people in the community who do not live in our buildings.

All help is tailored to each individual’s requirements via a structured care and support plan that employs the Young Person’s Star model to enable each person to take responsibility of their lives and work towards reaching their particular objectives. Support is concentrated in eight areas:

  • Accommodation
  • Work and education
  • People and their support
  • Health
  • Decisions and behaviour
  • Rent and money
  • Practical life skills

MENTAL HEALTH

We recognise that finding appropriate accommodation with the correct support is a critical component of conquering mental health difficulties. The quantity of support we provide varies according to needs and might range from a few hours per week to full-time or intensive support over a short period of time to help reach a goal. We also promote as much peer support as possible, because mutual support and a shared journey motivate people to make changes and grow together.

Mental health is something to which we can all connect, and it is something for which we provide help in all programmes. Our specialist mental health services assist persons who have been diagnosed with a mental health issue, which can range from anxiety and panic disorders, bipolar disorder, depression and personality disorders

People we serve gain self-confidence and better emotional well-being as a result of self-development and collaborative support from staff on their journey to independence and recovery. We help people by adopting the Recovery Star methodology, which focuses on the following areas:
